We are partnering with a well-established healthcare organization focused on delivering high-quality, patient-centered care to older adults in the community. This part-time Dietitian role offers the opportunity to make a meaningful impact by supporting individuals in maintaining their independence, health, and overall quality of life. In this role, you will collaborate with a multidisciplinary team to assess nutritional needs, develop personalized care plans, and provide education to patients and their families. This position includes a mix of clinical and community-based work, including occasional home visits. Key Responsibilities Conduct comprehensive nutritional assessments to identify dietary needs and preferences Develop and implement individualized nutrition care plans Provide nutrition education and counseling to patients, families, and caregivers Support disease-specific nutrition management (diabetes, heart health, renal care, etc.) Promote wellness and encourage self-management through education and support Inspect meals for compliance with prescribed diets and quality standards Assist with menu planning, recipe development, and coordination with food vendors Collaborate with physicians, nurses, social workers, and therapists to provide integrated care Maintain accurate and timely documentation in the Electronic Health Record (EHR) Participate in team meetings, caregiver groups, and community events as needed Ensure compliance with all state and federal regulations, including HIPAA Minimum Qualifications Active Oregon Licensed Dietitian (LD) credential Bachelor’s degree in Dietetics, Nutrition, Food Systems Management, or related field Valid CPR certification Valid driver’s license Desired Qualifications Experience working with elderly or medically complex populations Background in community-based care, home health, or long-term care settings Familiarity with interdisciplinary team (IDT) models of care Strong communication skills with patients, families, and care teams Contact Apply on our website or reach out via email to learn more! #J-18808-Ljbffr Eatrightoregon
