About the Role

As a Nurse Practitioner or Physician Assistant, you'll conduct annual in-home assessments, identify care gaps, educate patients, and collaborate with primary care providers. This is a non-prescribing, field-based position focused on preventive care.

Qualifications

• NP: ANCC or AANP certification
• PA: NCCPA certification and state licensure
• Active, unrestricted license
• Valid driver's license and reliable transportation

Preferred

• 1+ years of clinical experience
• Home health, primary care, or geriatric experience
• EMR proficiency

• Location: Yakima, WA
• Schedule: Flexible 24-40 hours/week
• Work-Life Balance: No on-call, weekends, or holidays
