This leadership role is responsible for overseeing the delivery of high-quality patient care, supporting and developing clinical staff, ensuring regulatory compliance, and partnering with interdisciplinary teams to achieve exceptional patient outcomes.If you are a compassionate leader who enjoys mentoring clinicians, building strong teams, and making a meaningful impact in the lives of patients and families, we'd love to meet you.What You'll DoProvide leadership, supervision, and support to a team of home health clinicians.Coordinate the delivery of high-quality patient care while promoting positive patient outcomes.Ensure compliance with all federal, state, local, Medicare, and Medi-Cal regulations.Conduct and facilitate interdisciplinary care conferences, team meetings, and clinical in-service education.Review referrals and coordinate timely patient assessments and admissions.Make appropriate patient care assignments while overseeing clinician scheduling and on-call coverage.Monitor clinical documentation and quality standards to ensure regulatory compliance.Maintain accurate records related to patient assignments and clinical management.Serve as a clinical resource, mentor, and preceptor for nursing staff when needed.Foster a collaborative, supportive culture focused on excellence in patient care.What You'll BringCurrent California Registered Nurse (RN) license in good standing.Graduate of an accredited school of nursing.Minimum of two (2) years of clinical nursing experience within the past five (5) years.Previous home health leadership or supervisory experience preferred.Strong knowledge of Medicare Conditions of Participation and home health regulations.Excellent communication, organizational, and leadership skills.Ability to build positive relationships with patients, families, referral partners, and team members.Valid driver's license, reliable transportation, and current automobile insurance.We believe our people are our greatest strength.
