We're seeking a motivated and compassionate Driving CHHA / HHA to join our team in the Westfield, NJ area. As a certified Home Health Aide, you'll be responsible for providing high-quality care to our clients in their own homes. Responsibilities:

• Provide exceptional care to our clients, including assistance with daily living activities, medication management, and personalized care plans

• Develop strong relationships with clients and their families, ensuring their needs are met and their expectations are exceeded

• Work collaboratively with our care management team to ensure seamless transitions and coordination of care

• Maintain accurate records and documentation, adhering to regulatory requirements and company standards

• Participate in ongoing training and education to stay current with industry best practices and company policies

Requirements:

• Must be a CHHA / HHA (Certified Home Health Aide) in New Jersey (License Must Be Current)

• Ability to pass a Background check

• Have current TB Clearance or the ability to pass the Two-Stage TB Test
