Bloom Healthcare is seeking a Registered Nurse Case Manager to join our San Antonio hospice team. The RNCM will coordinate hospice care, educate patients and families, and work with the interdisciplinary team to meet physical, emotional and spiritual needs in home or community settings. The ideal candidate has a TX RN license, at least one year in med-surg, emergency, or critical care, and strong assessment and communication skills. #J-18808-Ljbffr Bloom Hospice
