Unit Manager, LPN job summary:

• Monday - Friday; flexible hours (primarily day shift)

Sky View Rehabilitation and Health Care, a 192-bed skilled nursing facility located in Croton-on-Hudson, NY has an opening for an LPN Unit Manager. Sky View serves as the bridge between hospital and home, spanning the gap from recovery to community reentry. Our luxurious and hotel-like environment is the ideal setting for those in need of post acute rehabilitation or long-term care. Our team of highly skilled physical, occupational, speech, and respiratory therapists really care and work together to create an individualized therapy plan that helps residents reach their highest level of functioning in as short an amount of time as possible preparing for their eventual return home.

Unit Manager, LPN responsibilities include:

• Professional supervision, guidance and direction to staff in both clinical and personnel issues on the long-term or short term unit.
• Coordinating resident care in accordance with state DOH regulations.
• Providing support to residents/families in dealing with clinical issues and challenges.
• Ensuring staff adheres to safety and infection control guidelines.
• Assisting in establishing daily operations of resident care on unit. This includes but not limited to showers/baths, toileting, meals, medication administration and activities.
• Communicating to other members of the interdisciplinary team pertinent information to assure resident/family needs are met.
• Performing daily rounds of all residents on the unit.
• Performing assessments on residents as required by the facility - admission, elopement, pain, etc.
• Developing plan of care for each resident. Documenting and communicating plan of care on resident information sheets.
• Ensuring assessment: Documenting clinical observation and assessment on all assigned residents per regulations. Reporting findings of ongoing observations/assessments to the designated nursing supervisor. Documenting effectiveness of plan of care.
• Preparing resident assignments for certified nursing assistants, considering the category of needs of the patient and competency of personnel.
• Communicating ongoing resident, family and facility plan/concerns/information to staff.
• Organizing, overseeing and supervising assigned unit activities and staff assignments. Providing a nursing assistant assignment for each CNA on his/her team. Communicating to CNA pertinent resident care needs. Monitoring and evaluating the flow of activity and delivery of patient care provided by the CNA's (turning and positioning, mealtimes, bathing, etc.) Ensuring CNA documentation records (ADL's BM, meal intake, etc.) are completed.
• Communicating to the interdisciplinary team members any pertinent resident findings.
• Assisting other staff members in completion of their duties. Orienting new nursing personnel as assigned.
• Performing other related duties as required and assigned.

Requirements:

• Must be a graduate of an approved school of nursing.
• Current licensure in NYS as a licensed practical nurse.
• Minimum 3 years of skilled nursing experience.
• Minimum three (3) years' experience in a supervisory capacity either in a hospital, long term care facility, or other related medical facility, training in rehabilitative and restorative nursing practice.
• Must have a sincere desire to work with the elderly, chronically ill, and disabled.
• Must have a thorough knowledge of medical, social, and psychological needs of residents.
• Must utilize good judgment in emergency situations.
• Must possess a spirit of cooperation and enthusiasm in order to create an atmosphere conducive to rehabilitation and growth.
• Must be able to communicate well verbally with residents, families and staff.
• Must be able to handle emergencies with calm and intelligent response.
• Must have computer literacy.
