Job Summary:

Bertrand Chaffee Hospital in Springville, New York is seeking a dedicated and compassionate Physical Therapist to join our team. Our facilities offer a range of services, including inpatient discharge assessment and outpatient therapy, to promote healing and wellness. We are seeking a motivated and experienced Physical Therapist to provide high-quality patient care and contribute to our mission of "Keeping Healthcare Local" .

Responsibilities:

• Evaluate and treat outpatient population with a variety of physical disabilities and injuries
• Evaluate inpatient population appropriate for discharge
• Develop and implement individualized treatment plans to achieve patient goals
• Conduct physical therapy sessions, including exercises, stretching, and modalities
• Communicate effectively with patients, families, and healthcare team members
• Document patient progress and maintain accurate records
• Participate in interdisciplinary team meetings and contribute to patient care planning
• Act as liaison with schools for clinical education programs.
• Assumes responsibilities of Rehab Services Manager in her/his absence.

Requirements:

• Active New York State Physical Therapy license
• Masters degree or higher in Physical Therapy
• Minimum of 1 year of experience as a Physical Therapist
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced team oriented environment
