Licensed Practical Nurse (LPN) 4-Week Per Diem Contract

Position Summary

The Licensed Practical Nurse (LPN) provides high-quality nursing care to residents in a long-term care/skilled nursing facility during a 4-week temporary contract assignment . The LPN is responsible for providing safe, compassionate, and clinically appropriate care while working collaboratively with residents, families, physicians, registered nurses, certified nursing assistants, and the interdisciplinary team.

This position is designed for experienced LPNs seeking a short-term contract assignment with a guaranteed minimum weekly schedule.

Contract Details

·         Contract Length: 4 weeks

·         Position Type: Temporary / Per Diem Contract

·         Minimum Hours: 40 hours per week

·         Weekly Compensation: $1,600 per week, less applicable taxes and withholdings

·         Schedule: Flexible scheduling based on facility needs; must be available to work a minimum of 40 hours per week

·         Weekend Requirement: Every other weekend required

·         Shift: Day, evening, or overnight shifts based on facility needs and availability

·         Overtime: Subject to facility policy and applicable federal and state wage laws

·         Contract Completion: Assignment concludes at the end of the four-week term unless an extension is mutually agreed upon

Essential Duties and Responsibilities

Resident Care

·         Provide direct nursing care to residents in accordance with established care plans, physician orders, and facility policies.

·         Administer medications and treatments accurately and safely.

·         Monitor residents for changes in condition and promptly report significant findings to the RN, provider, and appropriate members of the interdisciplinary team.

·         Perform treatments, wound care, assessments, and other nursing interventions within the LPN's scope of practice.

·         Assist with admissions, transfers, discharges, and other resident-care processes as assigned.

·         Maintain accurate and timely documentation in the electronic medical record.

·         Promote resident dignity, privacy, independence, and quality of life.

Clinical Responsibilities

·         Follow all physician orders, nursing standards, facility policies, and applicable regulatory requirements.

·         Complete required assessments and documentation within established timeframes.

·         Participate in care planning and communicate relevant resident information to the interdisciplinary team.

·         Recognize and respond appropriately to changes in resident condition.

·         Follow infection prevention and control procedures.

·         Maintain medication and treatment areas in accordance with facility standards.

·         Participate in emergency response procedures as required.

Teamwork and Communication

·         Work collaboratively with CNAs, RNs, providers, rehabilitation staff, social services, dietary, activities, and other departments.

·         Provide appropriate direction and support to nursing assistants within the LPN's scope of responsibility.

·         Communicate professionally with residents and families.

·         Maintain a respectful and positive working relationship with facility staff.

·         Participate in shift report and ensure appropriate communication of resident needs.

Minimum Qualifications

·         Current, unrestricted LPN/LVN license in the state of employment.

·         Graduate of an approved practical nursing program.

·         Current CPR certification, if required by facility policy.

·         Previous long-term care, skilled nursing, rehabilitation, or geriatric nursing experience preferred.

·         Ability to work a minimum of 40 hours per week throughout the four-week contract.

·         Ability to work assigned shifts, including weekends as required.

·         Ability to successfully complete required employment screening and facility onboarding requirements.

Performance Expectations

Contract LPNs are expected to:

·         Maintain reliable attendance and punctuality.

·         Complete all scheduled shifts unless appropriately excused.

·         Provide safe, competent, and compassionate resident care.

·         Follow all facility policies and procedures.

·         Maintain professional conduct at all times.

·         Complete documentation accurately and timely.

·         Communicate changes in resident condition promptly.

·         Maintain confidentiality of resident and facility information.

·         Meet all applicable state and federal nursing requirements.

Contract Agreement

The LPN acknowledges that this is a four-week temporary contract assignment requiring a minimum of 40 hours per week . Compensation is $1,600 gross per week, less applicable taxes and withholdings , based on completion of the required minimum weekly hours and satisfactory performance.

Continuation beyond the four-week term is not guaranteed and may be offered based on facility staffing needs, performance, and mutual agreement between the LPN and facility.

The facility reserves the right to modify scheduling, assignments, and duties as necessary to meet resident-care and operational needs, consistent with applicable law.
