Job TitleYour Experience Matters – New Higher Salaries Based on Experience Hourly pay range: $26.00-$29.00As directed by the RN, assists with the monitoring and coordination of the delivery of health care services for individuals with developmental disabilities who are in a structured day program.Essential Functions/ResponsibilitiesAssists with monitoring and coordinating the delivery of preventive routine health care, therapies and programs for individuals with developmental disabilitiesAdheres to and assists with ensuring facility compliance with all Physician, NP, PA, etc. ordersAssists with coordinating medical program needs with community services, medical providers and other related agenciesAssists with ensuring adequate completion of health related exams, appointments, laboratory/diagnostic testing as ordered or requiredSafely transports individuals to and from appointments and community outings if necessaryAssists with the monitoring of medication administration in the facilityAdministers medications, treatments and supplements as assignedAssists with providing health services training for staffParticipates as an active member of the Interdisciplinary TeamMaintains compliance with and assists with ensuring facility compliance with all applicable State/Federal regulations and agency policy and procedureMaintains communication with Medical Liaison, Program Manager, Program Administrator, Service Coordinator and other staff as necessaryMonitors that transportation and all medical specific procedures are being performed adequately by direct care staffPerforms CPR and Strategies for Crisis Intervention and Prevention (SCIP-R)Complies with all agency policies and proceduresOther duties as requiredMinimum QualificationsCurrent NYS licensure to practice as a Licensed Practical NurseNYS Drivers License that meets agency policyAbility to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbingLifting requirements of 50lbsAbility to lift and transfer individuals manually and mechanically as required by prescribed program/planAbility to move, reposition, and place individuals in pieces of equipmentPhysical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelinesCPR certification requiredWhy People Inc.?
