Case Manager, Registered Nurse

CVS Health is hiring a Case Manager, Registered Nurse to provide field-based care coordination for diverse patient populations. In this role, you will conduct in-home assessments, develop holistic care plans, and collaborate with providers, pharmacies, and payers to improve outcomes and close care gaps. You’ll educate patients and caregivers, address social determinants of health, and ensure appropriate use of services.
