Service Coordination Supervisor

Description STARTING PAY: commensurate with experience

LICENSE/CERTIFICATION REQUIRED:

Service and Support Supervisor Certification – Franklin County Board of Developmental Disabilities (can be obtained upon hire); valid Ohio Driver's License - Ohio Bureau of Motor Vehicles; must meet and maintain Board/Ohio School Plan insurability standards for driving

DUTIES/RESPONSIBILITIES:

Supervises assigned employees through direct observation, regularly scheduled supervisory sessions and review of records; routinely reviews Service Coordinators’ progress notes and other written documentation; provides/coordinates orientation and training; rates probationary and annual performance evaluations; recommends leave requests; assists Director of Service Coordination in counseling assigned employees related to performance; Coordinates, supervises and ensures the provision of quality services to individuals with developmental disabilities Provides services to clients and families as needed and as back up for assigned employees; advocates for client rights and needs; refers clients to appropriate providers/agencies as needed; assures compliance with all relevant laws, rules, statutes, policies and procedures; Attends or coordinates assigned committees, planning groups or task forces; assists in Conflict Resolution Process; serves as a liaison between Service Coordinator and clients or community agencies as necessary Develops Service Coordination policies and procedures and updates as needed; completes necessary paper work, reports and documentation; Assists Director of Service Coordination in crisis situations as needed; serves as on-call staff Oversees Service Coordination residential functions including frequent interactions with Creative Housing and community housing partners.
