Care Transitions Navigator

The Care Transitions Navigator maintains ongoing contact with health care organization, insurance companies, and the medical community to facilitate home care services, specialty medications, financial assistance programs for medication assistance, and streamlines the transition of patient care between facilities and care providers within CoxHealth 340B clinics. The Navigator will work as a liaison between patients, clinic staff, and the Prior Authorization Team. This role may involve occupational exposure to hazardous drugs. Training and education will be provided on any specific procedures, risks, and safety protocols associated with the handling of these drugs.

Education Required: Graduate of an approved nursing program

Experience Preferred: Minimum one (1) year experience as a professional nurse within the last three (3) years. Preferred: Six (6) months home health experience.

Skills Knowledge of Medicare and Medicaid regulations desirable.

Licensure/Certification/Registration Required: MO RN License
