Charge Nurse - LVNThe Charge Nurse - LVN/LPN supervises the day-to-day nursing activities of facility or unit within facility in accordance with federal and state regulation, Company policies and procedures and nursing standards so as to provide the highest quality of nursing care to our residents.Duties and responsibilities include:Directing and supervising day-to-day functions of assigned personnel and ensuring that policies and procedures are followedOverseeing implementation of resident plan of care and evaluating resident responseCompleting routine rounds during shift to observe residents, to determine if nursing needs are being met and to ensure nursing personnel are performing work assignments in accordance with acceptable nursing standards, facility policies and procedures, and governmental regulationsInitiating emergency support measures as neededMonitoring treatment schedules to ensure they are provided as scheduled and may perform duties of Treatment NurseCompleting requisitions and arranging for diagnostic and therapeutic services, as ordered by physician, and in accordance with facility's established proceduresContributing to formulation/review of nursing care plans weekly and PRNProviding resident/caregiver instruction and education as necessaryEncouraging independence by resident in activities of daily living to fullest extent possibleReceiving and giving change of shift report upon reporting in and ending shiftPerforming documentation responsibilities in accordance with company requirementsPreparing and administering medications as ordered by physician in accordance with governmental regulations and facility policies and proceduresCoordinating with resident's physician in the care of resident including notification upon change of conditionMaking rounds with physicians as necessaryIdentifying resident safety concerns and taking appropriate actions to maintain a safe environmentPerforming other related duties as requiredKnowledge, skills, abilities, and qualifications include:Nursing degree from accredited college or graduation from approved LVN programLPN/LVN license requiredValid Adult Basic First Aid and CPR Certification preferredA minimum of 1-3 years of experience in direct patient care, preferably in acute or long-term care setting, with responsibility for providing nursing care to multiple residents preferred; previous Charge Nurse or supervisory experience preferredAlign with the Organization's mission and values, delivering high-quality, person-centered care with dignity, respect, compassion, and integrityAbility to interact professionally with residents, families, staff, and the public, and handle challenging situations with respectUnderstanding and adherence to relevant regulations, policies, and proceduresEffective team member with strong communication and cooperation skillsAbility to organize, prioritize, and meet deadlinesFollow instructions, accept feedback, and function well in a dynamic environment
