Conduct comprehensive assessments of patients and document health status changes. Develop, implement, and evaluate individualized care plans for residents. Supervise and guide nursing assistants and other staff. Ensure proper documentation of resident care and maintain accurate medical records. Communicate effectively with families, physicians, and the healthcare team to coordinate care. Uphold facility policies, procedures, and compliance with state and federal regulations. Qualifications High school diploma or equivalent. Registered Nurse license in Ohio. Strong customer‑service skills. Previous LTC/SNF experience preferred. Serious applicants only.
