Position Summary

The facility is seeking a Board-Certified or Board-Eligible Family Medicine Physician to join its outpatient primary care team in Charleston, South Carolina .

This role includes:

• Outpatient primary care for patients of all ages

• Chronic disease management (HTN, diabetes, COPD, thyroid disorders, etc.)

• Preventive care, annual wellness visits, and health screenings

• Acute care visits and same-day appointments

• Basic office procedures depending on skill and interest

• Collaborative care with specialists, APPs, and care coordinators

Essential Position Responsibilities

• Provide comprehensive outpatient primary care to pediatric, adult, and geriatric patients

• Diagnose and manage acute and chronic medical conditions

• Perform preventive care visits, annual physicals, and wellness examinations

• Order, review, and interpret laboratory, imaging, and diagnostic studies

• Develop individualized treatment plans and provide ongoing follow-up care

• Perform minor office-based procedures when applicable

• Collaborate with APPs, nursing staff, and multidisciplinary care teams

• Coordinate care with specialty providers and ancillary services

• Maintain accurate and timely documentation within the EHR

• Participate in quality improvement initiatives and team meetings

QualificationsEducation

• M.D. or D.O. degree

• Completion of an accredited Family Medicine residency program

Licensure & Certification

• Board Certified or Board Eligible in Family Medicine

• Eligible for South Carolina medical licensure

• BLS certification required

Experience

• Open to new residency graduates

• Experienced Family Medicine physicians encouraged to apply
