Job Description
Skilled Home Health RN

Schedule

Day shifts typically from Monday to Friday, options include 8:00 AM to 5:00 PM or 9:00 AM to 6:00 PM. Some block schedules available. Alternating weekend coverage is required. Visits are self-scheduled based on office needs.

Assignment Details

Visits per day: 6-8; orientation is approximately 40 hours.

Requirements

• Minimum 1 year of recent clinical experience
• Skilled home care experience required; private duty experience not accepted
• OASIS training and experience required
• Must have recent patient care experience within the last 3 months
• Valid driver's license and reliable transportation required for home visits
• Pass/fail EKG exam required (up to 3 attempts)
• COVID-19 vaccine required (medical/religious exemptions allowed)
• Influenza vaccine or declination required; mask required while at work

Responsibilities

• Conduct comprehensive assessments of patients in their homes
• Administer medications and treatments as prescribed
• Provide patient education on self-care techniques
• Implement care plans and document patient progress
• Collaborate with healthcare team members to optimize patient outcomes
