Campbell Hall Rehabilitation Center is seeking a compassionate, motivated, and licensed Physical Therapist to join our interdisciplinary rehabilitation team. Our long‑term care and skilled nursing facility is committed to providing exceptional rehabilitative care that enhances our residents’ independence, mobility, and quality of life. As a Physical Therapist, you will work collaboratively with physicians, nursing staff, occupational therapists, speech‑language pathologists, and other healthcare professionals to develop and implement individualized treatment plans that help residents achieve their highest level of function. Responsibilities Evaluate residents’ physical abilities, limitations, and rehabilitation needs. Develop individualized treatment plans based on physician orders and clinical assessments. Provide therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence. Assess and recommend appropriate assistive devices and adaptive equipment. Educate residents, families, and caregivers on treatment plans, safety, and home exercise programs. Maintain accurate and timely documentation in accordance with facility policies, Medicare, Medicaid, and state and federal regulations. Participate in interdisciplinary care plan meetings and discharge planning. Ensure compliance with all infection control, safety, and regulatory standards. Collaborate with the rehabilitation team to achieve positive clinical outcomes. Qualifications Current New York State license as a Physical Therapist. Graduate of an accredited Physical Therapy program. Previous experience in a skilled nursing facility or long‑term care setting is preferred but not required. Knowledge of Medicare, Medicaid, PDPM, and rehabilitation documentation is preferred. Excellent communication, organizational, and clinical skills.
