TITLE: Case Manager

DEPARTMENT: Community Programs 

DIVISION: Human Services

RESPONSIBLE TO: Program Coordinator/Director of Community Programs

SUPERVISES: No direct reports

FLSA STATUS: Non-Exempt

SCOPE:

Develop and monitor the implementation of Personal Development Plans (PDP) with participants in an assigned program, provide guidance and support to participants to meet program objective; and contribute to the mission of Goodwill by advocating for the participants and maximizing opportunities for persons with disabilities and other barriers to become more independent, guided by precedent and working within the limits of established policies.

ESSENTIAL FUNCTIONS:

1. Receive and review case files for new participants in the assigned program. Meet with individual participants and review interests, work history, and goals. Identify the assets and barriers of participants through observation, interviews, case notes and other means. Assess participants’ abilities to learn skills for reintegration into the community, job readiness and participate in daily life enrichment activities. Work in cooperation with participants and referring agencies to establish individual participant goals for all relevant programs.

2. Define the process for attainment of goals with measurable step by step objectives. Document objectives and process for attainment as a PDP and present to management for approval. Ensure that PDP is in compliance with all regulations and guidelines.

3. Make regular assessments of participant’s progress against objectives and document results. Review the goals and update with progress and program changes. Complete quarterly progress reports, annual reports, and all other required documentation mandated by Agency for Persons with Disabilities/Qlarant. Report and document all Incident Reports and Follow-Up Incident Reports internally and to Agency for Persons with Disabilities as required and based on category (critical vs. non-critical). 

4. Provide participant with intervention counseling and support; and serve as a liaison between program providers, direct care staff, and other support providers. Provide crisis intervention and advocacy, assessments, and information gathering and sharing with staff. Research and recommend resources and make referrals to other programs. 

5. Work in cooperation with the direct care staff to establish processes for all relevant programs to achieve participant goals. Assist staff with interaction and intervention with participants during activities or outings. Observe the participant, intervene and collect input from direct care staff. Assess the participant’s advances toward community reintegration and job readiness. May assist in finding community apprenticeships and/or internships.

6. Communicate with employment consultant and job coach. Participate in the job search when relevant. Participate in and provide skill development opportunities in resume development, completion of job application, transportation arrangements, practice interviews, and other pre-employment skills and activities.

7. May participate in and facilitate support groups. Provide materials, plan workshops and special events, and engage speakers. Serve as a liaison between program and other support agencies. Attend support plan meetings with family to address progress and changes in goals or participant requirements, as appropriate.

8. Ensure that the case files and participant records are accurate and complete by periodically updating all required forms. Ensure file is in compliance with regulations and requirements. Enter participant updates and information into databases in accordance with policy and procedures. Collect and calculate statistics by participant and submit to referring agencies as periodically required.

9. Document participant involvement with case plan. 

10. Review and collaborate with Waiver Support Coordinators (WSC) on Service Authorizations to determine program attendance; follow up on billing questions as needed and assist Program Coordinator on Aged Receivables. 

11. Participate in staffing with participants, families, WSCs, and other advocates; review and discuss behavior plans with participant supports i.e.: Behavior Analysts. 

12. Meet periodically with participant staff team members to update goal requirements and assess progress; complete worksheets to reflect current goal set and assessment criteria. 

13. Develop and conduct participant trainings based on program curriculum as needed. 

14. Transport program participants to and from program and outings as needed. Observe and monitor the safety of program participants, report any abuse/neglect suspicions, and maintain the 1:10 ratio. 

15. Attend all required internal/external trainings as assigned. 

16. Participate in training and gain knowledge on the data systems used to document participant activities; APD I connect, Client Track and internal ADT NET system

17. Provide assistance with other general department activities.

1. Demonstrate by words and actions a commitment to the Goodwill mission to help people with disabilities and other barriers to become self-sufficient, working members of the community.

2. Perform or assist with any duties or operations, as required to maintain workflow and to meet schedules and quality requirements.

3. Maintain safe work area and comply with safety procedures and equipment operating rules, keeping work area in a clean and orderly condition.

4. Participate in any variety of meetings and task force groups to integrate activities, communicate issues, obtain approvals, resolve problems and maintain specified level of knowledge pertaining to new developments, requirements, and policies.

Requirements:

KNOWLEDGE SKILLS AND ABILITIES:

1. Bachelor’s Degree in Rehabilitation, Social Services, Human Services or related field. Relevant work experience may be substituted on a year for year basis in lieu of degree. 

2. Minimum of one (1) year case management experience preferred.

3. Must have CPR and First Aid certification. 

4. Proficiency in Microsoft Office Suite.

5. Excellent communication skills required. Ability to establish and maintain rapport with employers and referral agents.

6. Excellent problem-solving skills and organizational skills.

7. Must have a valid Florida driver’s license, valid insurance, reliable transportation for travel to outlying locations and the ability to be insured under the company’s vehicle insurance policy.

PHYSICAL REQUIREMENTS:

1. General office environment

2. Occasional lifting and/or carrying up to 30 lbs.

3. Travel for community outings with participants and may be asked to work at either location of the Capabilities Adult Day Training Programs

 

TOOLS AND EQUIPMENT USED:

Computer and usual peripherals, word processing, spreadsheets and software programs, and standard office equipment, automobile, large passenger van, or wheelchair accessible van, as required.
