- MD or DO degree.
- Board Certified or Board Eligible status.
- Active and unrestricted medical license.

Educational Background- Required: Doctor of Medicine (MD).
- Preferred: PhD.
- Experience in Family Practice is essential.
- Valid Driver's License.
- Licensed Physician as per the Board of Medicine.
- Drug Enforcement Administration (DEA) License.
