Job Description

Job Summary:

The TRAVEL - RN CASE MANAGEMENT position supports inpatient and transitional care through utilization management, care coordination, and discharge planning. This role partners with the interdisciplinary team to ensure timely, appropriate care and accurate documentation within the hospital's EMR environment.

Responsibilities:

• Manage utilization by reviewing clinical needs and coordinating care to support appropriate level-of-care decisions.
• Coordinate patient services across disciplines to streamline workflows and reduce delays in care transitions.
• Conduct discharge planning to ensure continuity of care, services, and resources upon transition.
• Maintain revenue integrity through accurate documentation and adherence to clinical and administrative requirements.
• Collaborate with clinical teams to support care plans, referrals, and problem resolution for complex cases.
• Utilize EMR tools to document patient status, progress, and care coordination activities.

Qualifications:

• Hold an active NH RN license at time of submission.
• Hold an active Compact RN license at time of submission.
• Maintain BLS certification.
• Possess 2 years of hospital experience, with strong alignment to acute care case management needs.
• Demonstrate critical care experience.
• Exhibit leadership qualities and professional, business-appropriate conduct.
