Consults with medical and professional staff to plan and coordinate joint patient management objectives. Acts as a case manager for patients when primary need is for physical therapy. Has contact with patients under a variety of circumstances. Is subject to varying and unpredictable situations and occasionally to irregular hours.***EDUCATION*** * Bachelor of Science or Master’s degree in physical therapy from a program accredited by the American Physical Therapy Association (APTA), required.***EXPERIENCE**** Two years of experience as a physical therapist required.* One year of experience in providing rehabilitative therapy to a variety of patients including the elderly, required.* Community/home health experience preferred.***LICENSURE/CREDENTIALS**** Licensed to practice as physical therapist in the State of Washington required.* Current BLS certification required.* Valid Washington driver’s license required.* Proof of insurance on automobile used for work required.* Driving record in compliance with OMC standards currently in effect or as may be amended, required.
