Hospice Registered Nurse Case Manager

Care 360 Hospice is seeking a Hospice Registered Nurse Case Manager to join our Dayton area team. The RNCM will assess patients, develop care plans, coordinate with multidisciplinary teams, and provide on-call coverage as required. Travel to patients' residences is required; valid RN license and BLS are essentials.
