Nurse Coordinator

Providing care coordination for Medicaid and Medicare members, the full-time Remote Bilingual Nurse Coordinator will build trusted relationships, assess member needs, and facilitate access to care while working from a dedicated home office in the Central Time Zone. Key Responsibilities Conduct outreach to educate members about program benefits and assess their status to develop personalized care plans Manage a caseload of members, ensuring timely communication and coordination of care with providers Document actions taken in a care management platform and identify needs related to Social Determinants of Health Required Qualifications Valid and unrestricted Licensed Practical Nurse (LPN) or Registered Nurse (RN) license, multi-state compact preferred Fluency in Spanish and English Minimum of 2 years of experience in care coordination or related fields 1+ years of remote work experience Experience working with Medicaid and/or Dual Eligible patients
