Part-Time Geriatric NP: Primary Care & Home Visits

PACE

PACE SEMI seeks a part-time Nurse Practitioner to provide primary care across clinic, home, and telemedicine settings in Westland, MI. The NP will manage chronic diseases, perform annual reviews, and support care transitions under supervision of the Physician-In-Charge. The role emphasizes geriatric care, coordination with the interdisciplinary team, and documentation within 24–72 hours.
