Clinical Social Worker 2

Under the direction from the supervising social worker, the incumbent will work with individuals living with HIV, those affected by HIV, and families to provide comprehensive social work services within an ambulatory care setting. These services include, but are not limited to, psychosocial assessments; assisting patients in referrals to emergency and long‑term housing stabilization, substance use counseling, mental health assessments; developing and administering behavioral interventions; and making appropriate referrals to community agencies for financial benefits, legal, and immigration resources. The incumbent will also provide crisis and behavioral interventions, provider consultations, insurance, and financial counseling, and participate in multidisciplinary case conferences for care‑plan development. Participation in social work meetings, staff meetings, case conferences, and other Ward 86 Clinic meetings is required. Responsibilities Ambulatory Care Social Services (85% of time) Track healthcare utilization, schedule appointments, make reminders, and follow up on missed appointments. Perform psychosocial intakes and assessments for new and existing patients, including comprehensive substance use and mental health evaluations; develop and administer interventions; evaluate outcomes and support systems. Provide one‑on‑one counseling for substance use, mental health, and overall wellbeing. Conduct de‑escalation and crisis management as needed, including involuntary holds and Tarasoff procedures. Assess patients for interpersonal or domestic violence, elder and child abuse; make appropriate referrals, file police reports, and participate in safety planning and crisis intervention. Assist patients in accessing financial benefits and money management. Refer patients to community‑based case‑management agencies, substance use programs (residential and outpatient), mental health, psychiatry, and psychotherapy services. Support patients with transitional and permanent housing options through referrals to stabilization shelters, Hummingbird, HIV emergency stabilization rooms, and permanent housing waitlists. Provide comprehensive services to patients with complex psychosocial issues who present without scheduled appointments, including navigation of systems and access to benefits. Perform other general social work duties as assigned. Other Duties (15% of time) Attend staff meetings, social work meetings, and training sessions. Maintain patient records in the Ward 86 electronic medical record system and other required documentation. Maintain collaborative relationships with social workers and other community service providers. Participate in the Ward 86 QI initiatives and perform other assigned duties. Required Qualifications Master’s degree from an accredited school of social work and possession of the requirements necessary for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners; or an equivalent combination of education and experience; and knowledge and abilities essential to the duties assigned. Active license with the California Board of Behavioral Sciences as an LCSW, ASW, or equivalent. Two to four years of social work experience providing services to HIV‑positive populations diverse in gender, sexual orientation, language, country of origin, race, and ethnicity. Ability to work in a high‑stress, high‑volume area and perform well under pressure and with deadlines. Ability to handle a demanding workload involving multiple tasks effectively and efficiently. Ability to function independently while being a collaborative and supportive team member. Demonstrated cultural humility and a strengths‑based, recovery philosophy. Excellent oral, telephone, and written communication skills and documentation. Computer‑literate with working knowledge of HER systems and Microsoft Suite. Excellent organizational and interpersonal skills. Demonstrated excellent attendance and reliability. Preferred Qualifications HIV Test Counselor. Licensed Clinical Social Worker or Licensed Marriage–Family Therapist. Fluency in Spanish and ability to communicate effectively with monolingual patients. Required Licenses and Certifications Active license with the California Board of Behavioral Sciences as an LCSW, ASW, or equivalent.
