Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, utilizing evidence-based practices to identify health needs.
• Diagnose and manage a wide range of acute and chronic medical conditions, providing preventive care tailored to each patient's specific requirements.
• Work collaboratively with community organizations to design and implement impactful health education programs that empower individuals and promote health literacy.
• Advocate for patients by ensuring their rights are respected while facilitating their access to necessary healthcare services.
• Coordinate referrals and follow-up care, ensuring that patients receive comprehensive treatment and support as needed.
• Maintain detailed medical records in compliance with regulatory standards, ensuring accurate documentation of patient interactions and care plans.
• Participate in community outreach events and health fairs, engaging with the local population to raise awareness about available health resources.
• Stay current with emerging trends, best practices, and policies in community health to continually enhance service provision.

Patient Population / Client Focus

This role primarily focuses on serving a diverse patient population in the Watertown area, including individuals and families from various socioeconomic backgrounds. You will engage with patients of all ages, addressing a breadth of health concerns and promoting healthy lifestyle choices. Your culturally competent approach will ensure that all patients feel respected, understood, and empowered to take an active role in their health care.

Qualifications

• A Bachelor’s Degree in Nursing from an accredited institution.
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• DEA# and NPI# are required for this position.

Preferred Experience

• A minimum of 3 years of clinical experience, preferably in community or outpatient health settings.
• Demonstrated proficiency in primary care, family medicine, or public health.
• Strong clinical assessment, diagnostic, and decision-making skills.
• Exceptional communication and interpersonal skills, enabling effective interaction with diverse patient populations.
• Ability to thrive independently as well as part of a multi-disciplinary healthcare team.
• Proficiency in utilizing EHR systems to maintain patient records and coordinate care.

Compensation and Benefits

This position offers a competitive salary range of USD 150,000 to 160,000 annually, commensurate with experience and qualifications. In addition to a robust salary, Empower Health NP provides a comprehensive benefits package that includes health insurance, retirement plans, professional development opportunities, and a sign-on bonus of up to $10,000.
