Behavioral Health Case Manager - Complex Care & Coordination

CVS Health

CVS Health is seeking a Case Manager to support members enrolled in Medicare and Medicaid, focusing on complex health and social needs. You will coordinate care with providers, families, and community resources to promote wellness and effective outcomes. This at-home/field role requires travel up to 50-75% in the Southloop and surrounding areas, a Master’s in Behavioral/Mental Health, and Illinois licensure as LCSW or LCPC.
