ACT Program Liaison: Clinical Admin & Care Coordination

You will be working on an interdisciplinary Assertive Community Treatment (ACT) clinical team working with individuals diagnosed with a Serious Mental Illness or General Mental Health diagnosis under Court Ordered Treatment. This individual will liaison with clients' prescribing Clinician, Nurse and other members of the clinical team to ensure the timely development and implementation of the individual service plan. Experience working in clinical/behavioral health setting preferred.Ability to maintain professional working relationships.Certified Psychiatric Rehabilitation Practitioner (CPRP) certificate (preferred).Intermediate skills in Microsoft Word, Excel, and Outlook required. Experience in PowerPoint and electronic medical record - NextGen preferred.Experience with multi-line phone systems and ability to perform general math skills required.Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment.This role is a non-driving position. This position is performed at one location and does not require travel to various Terros Health centers.
