Collaborate with a multidisciplinary team to develop and implement patient-centered treatment plans that promote recovery and independence. Monitor patient progress, modify therapy as needed, and document outcomes thoroughly to ensure quality care and compliance. Qualifications 2 years of experience as a Speech Language Pathologist - CCC. Qualified applicants must have at least 2 years of experience in Home Care.
