Housing Stability Case Manager
Cornerstones
Cornerstones is seeking a Housing Focused Case Manager to provide linkages to and coordination of services for adults at the Embry Rucker Community Shelter (ERCS). The role focuses on rapid exits to permanent housing using a Housing First approach, with an emphasis on equity and trauma-informed service delivery. You will work with Cornerstones and ERCS teams to reduce homelessness length, support income growth, and address barriers to housing through individualized plans and collaborative care. #J-18808-Ljbffr
