About the Role: 

The Prevention Case Manager supports survivors in the community experiencing domestic and sexual violence, stalking, and/or human trafficking who are at imminent risk of homelessness.  This position is responsible for providing time-limited direct services to clients by assessing client needs, connecting clients to appropriate services, and advocating to ensure the provision of comprehensive services which support survivors in increasing safety and achieving housing stability. The Prevention Case Manager utilizes their knowledge of community resources to provide families with connection to legal services, case coordination, and other supportive services in the community as needed.  Case management services are provided in a client-centered manner driven by the victim’s needs and goals and respecting client self-determination. 

Responsibilities:  

• Provide crisis intervention to survivors of domestic and sexual violence and, in collaboration with the client, develop an individualized safety plan that promotes safety and housing stability 

• Perform assessment of client’s physical health, mental health, social needs, employability, educational needs, quality of life issues, strengths and needs, and other necessary areas 

• Administer flexible financial assistance to support client goals related to safety and stability 

• Provide mobile advocacy services by meeting with assigned clients regularly in a safe location in the community 

• Assist clients in filing for protective orders and completing affidavits, and accompany clients to court proceedings and various appointments (i.e., attorney meetings, protective order hearings, custody hearings, and housing interviews) 

• Coordinate services amongst mental health service workers, service providers, advocates, the legal system, and other involved parties 

• Provide domestic violence psycho education to clients utilizing tools such as The Power and Control Wheel and the Cycle of Violence 

• Coordinate children’s services including education and daycare, programming, and counseling 

• Maintain complete and accurate client records and all documentation in accordance with agency and best practice 

• Completely and accurately report client information into identified program data systems (VADATA, DV Database) 

• Assure all relevant information pertaining to clients is disseminated to all appropriate staff members in a timely manner 

• Collaborate with the Assistant Director for Domestic Violence Community Housing to ensure comprehensive client driven service delivery 

• Advocate for clients and complete referrals to meet client needs 

• Listen effectively, deliver effective written and oral communication 

• Maintain familiarity with agency and program emergency procedures 

• Participate in staff meetings and trainings as directed by supervisor 

• Function as a contributing member of a multidisciplinary team with Shelter House Inc. employees and involved agencies to assist client with their identified goals 

• Maintain compliance with all agency personnel policies 

• Proficiently utilize Microsoft products as a part of day-to-day work requirements.

• Perform other duties deemed necessary to support the program and agency 

About You: 

Required: 

• A bachelor's degree in social work/human services, related field or commensurate experience 

• 1+ years of case management/ direct services 

• Strong written and oral communication skills 

• Ability to prioritize competing priorities and make sound judgements 

• Ability to complete tasks while navigating frequent interruptions 

• Ability to deliver creative, resourceful solutions to unique challenges 

• Strong organizational and time management skills 

• Valid Driver’s License and reliable transportation / 10-15% local travel for appointments 

• Ability to Drive a minivan 

• Willingness to work evenings and weekends when needed 

Preferred: 

• Experience with providing services to victims of domestic violence, mental health issues, and substance abuse 

• Foreign Language fluency in a language other than English (ex. Spanish, Farsi, Arabic or Amharic) 

Physical Requirements: 

• Annual TB Test is required 

• Ability to sit or stand for long periods 

• Ability to lift items weighing 10-20 pounds
