RN Case Manager: Care Transitions & Discharge Specialist

Inova Fair Oaks Hospital is seeking an RN Case Manager II to develop and manage comprehensive patient care plans across the continuum. You will collaborate with physicians, nurses, and other care team members to ensure timely, appropriate patient management and effective discharge planning. The role requires Virginia RN licensure or eligibility, BLS certification, and at least 2 years in case management or clinical care.
