RN Case Manager: Discharge & Care Coordination

Inova Fairfax Hospital is seeking an Experienced Registered Nurse Case Manager 1 to join the Case Management Team. This full-time role runs Monday to Friday with rotating weekends and supports discharge planning and continuity of care in acute and post-acute settings. The RN Case Manager 1 coordinates services, liaises with patients, families and the interdisciplinary team, and uses utilization management to ensure appropriate, efficient care. #J-18808-Ljbffr
