Overview
Physical Therapist – General, travel assignment in a long-term care setting (SNF/ICF) in Hilo, Hawaii. Start date 09/14/2026 for a 13-week contract. Weekly pay range: $1,717 to $1,862 per week. Shift: Rotating; generally five days per week with potential weekend coverage. This role is in a skilled rehabilitation setting and requires Hawaii state licensure and professional practice.

Responsibilities
- Provide rehabilitation services in a SNF/ICF setting, performing evaluations, reassessments, and treatments within licensure and scope of practice.
- Develop, implement, and monitor individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team.
- Apply evidence-based therapeutic interventions to maximize function, mobility, communication, swallowing, and activities of daily living as applicable.
- Educate residents, families, and caregivers regarding treatment plans and patient progress.
- Complete accurate, timely, and compliant documentation; record progress, changes in plan of care, and discharges in accordance with facility, state, federal, and payer requirements.
- May supervise therapy assistants and be prepared for possible floating to other units or facilities based on census and staffing needs.

Requirements
- Current Hawaii license in good standing appropriate to the discipline
- Current Basic Life Support (BLS) certification
- Minimum two years of SNF/ICF experience preferred
- Ability to work a five-day schedule with potential weekend day coverage
- Must arrive the Thursday before start to complete employee health requirements
- Training: orientation to the rehabilitation department, EMR documentation (Net Health Rehab Optima and Epic), and facility workflows provided
