Job Summary
The Clinical Care Coordinator service adopts a "whole-person" approach to address the client’s needs related to physical health, behavioral health, and social determinants of health. CCC involves the coordination of care between the behavioral health clinician and the clinical personnel of an external provider (e.g., primary care, another behavioral health provider, hospital) Clinical Care Coordination occurs when the practitioner via direct face-to-face contact, video-conferencing, or telephone communicated treatment needs, assessments, and treatment information to external healthcare providers and facilitated appropriate linkages with other healthcare professionals, including transitions into or from a higher level of care or institutional settings. CCC also includes treatment planning and plan of care implementation activities that are separate from the diagnostic assessment service when the clinician and client are meeting face to face or via video conferencing. The Clinical Care Coordinator is responsible for ensuring that the client is at the appropriate LOC. If the clients fail to make progress or have met all of their treatment goals, it is the Clinical Care Coordinators’ responsibility to ensure timely assessment and transfer to a more appropriate LOC.

The key responsibilities of the Clinical Care Coordinator include but not limited to:
Duties and Responsibilities
- Facilitate outreach to establish referral providers and participate in outreach events
- Facilitate intake admissions and reassessments as scheduled
- Board approved and active licensure in the District of Columbia (assigned credentials outlined in Job Description)
- Prompt and regular attendance at an assigned work location.
- Ability to complete [or oversee the completion of] assigned tasks/projects in a timely manner.
- Carry a caseload
- Determine the frequency of ongoing Diagnostic Assessment and Plan of Care services
- Collect available social, educational, economic, and vocational information to be used as part of the assessment process
- Implement or revise patient treatment plans with the patient as required per state regulation and record a review of the patient’s progress according to BHG policy
- Develop within (7) days of entry or as required by state guidelines, whichever is more stringent, a formal treatment plan documenting patient goals and update utilizing the same guidelines
- Maintain patient records that are complete, comprehensive, and well organized
- Submit records for periodic review as requested by the supervisory team
- Document all patient contacts. If other team members provide services to the patient, the assigned counselor is responsible to see that such contacts are documented in the patient’s chart
- Make appropriate referrals through case-management services and perform necessary follow-up
- Assist the Clinical Director in implementing new programs into patient treatment
- Advise the Clinical Director of problems encountered with caseload management and schedules
- Attend weekly treatment team meetings. Be prepared to discuss his/her assigned individual patient caseload
- Hold a basic understanding of alcohol/drug abuse and addiction. Additionally, keep up- dated on current drug abuse and treatment information, especially current trends in Opioid Maintenance Treatment
- Provide intake/readmission services, including several weeks of supportive follow-up counseling for new patients until their assignment to a primary counselor
- Provide, monitor, and document follow-up calls/contact to all discharging patients for at least 30 days
- Provide crisis intervention/support services to patients assigned to other counselors when their primary counselor is unavailable
- Provide inter-agency reports/documentation required for third party reimbursement, criminal justice proceedings, supplemental income assessments, emergency services, and other specialized treatment needs of patients as applicable
- Coordinate care with other community providers as needed
- Other duties as assigned by supervisory team
Regulatory
- Ensure compliance with all federal and state regulatory bodies, as well as accrediting agencies
- Complies with all federal, state, and local regulatory agency requirements
- Complies with all accrediting agencies
- Meet BHG attendance standards and be present and ready for work at the beginning of their established work schedule
- Report any work-related injury or illness, workplace hazards, or security issues immediately to the supervisor
Marketing and Outreach
- Participate in community and public relations activities as assigned.

Minimum Requirements and Qualifications
The Clinical Care Coordinator must hold one or multiple of the following licensures: Physicians, psychologists, LICSW, LGSW, APRN, RN, LISW, LPC, LMFT, and LGPC. Also, must have the appropriate experience for the role. - Other treatment providers in the area
- Federal, state, and other local social service agencies.
- Determine resources needed to achieve results
- Well-versed on current drug abuse and treatment information-specifically the current trends in Opioid Maintenance Treatment
- Hold current knowledge of third-party payers’ billing and documentation requirements in the region
- Organize time and responsibilities to meet requirements of the workload
- Demonstrate basic knowledge and skill of office equipment and software
