Our providers deliver integrated clinical services including internal medicine, psychiatry, psychotherapy, and podiatry, helping facilities meet the complex medical and behavioral health needs of their residents.Position Summary:Vitae Health is seeking both full-time and part-time Psychotherapists to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents' overall well-being and adjustment to medical and life changes.Key Responsibilities:Conduct comprehensive psychosocial assessments for residents upon admission and as neededDevelop, implement, and monitor individualized treatment plansProvide individual, group, and/or family therapy sessionsAddress issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disabilitySupport residents coping with chronic illness, pain, loss of independence, and end-of-life concernsCollaborate with physicians, nursing staff, social workers, and rehabilitation teams to coordinate careParticipate in care plan meetings and interdisciplinary team discussionsMaintain accurate and timely clinical documentation in compliance with facility, state, and federal regulationsProvide crisis intervention and behavioral management strategies when neededEducate staff and families on mental health conditions and effective communication strategiesEnsure compliance with HIPAA and ethical standards of practice
