LPN Profession: Nursing Specialty: Long Term Care

Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by the certification. The individual must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the lifespan. Possesses the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs. Provides care needed by adult and geriatric patient groups. Resident's activities of daily living needs are met as outlined in the plan of care and in accordance with policies and procedures. Ensures a safe and comfortable environment, including hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care. Documentation is completed per protocol. Maintains resident's environment and safety as outlined in the plan of care and in accordance with policies and procedures. Answers call lights and alarms promptly and provides safety measures such as safety mats and low beds. Restraint use is under the direction of Nursing, ensuring cleanliness of the unit, behavior and fall prevention, and appropriate use and monitoring of malfunctioning equipment. Documentation is completed per protocol. Resident's nutritional needs will be met as outlined in the plan of care. Includes meal delivery, tray preparation, adaptive equipment, feeding, and honoring resident's preferences and choices. Documentation is completed per protocol. Resident's rehabilitative needs are met as outlined in the plan of care. This includes range of motion, ambulation, transfers, proper body alignment, and use of assistive devices. Documentation is completed per protocol. Reports resident's rights, refusals, and concerns to the Charge or Supervising Nurse in a timely manner in accordance with policies and procedures. Identifies and reports changes in resident's physical, emotional, and psychosocial status to the Charge Nurse promptly. Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
