A Home Health Scheduler coordinates patient visits by matching clinicians with patients, maintaining schedules, and ensuring timely, efficient delivery of home healthcare services.

QUALIFICATIONS

• Home Health Scheduling experience is REQUIRED .

• Bilingual Spanish (Preferred)

• Knowledge of medical terminology and home health operations is an asset.

• Proficiency with Microsoft Office and EMR/scheduling software (e.g., WellSky, Homecare Homebase, MatrixCare, or similar).

• Strong organizational, multitasking, and time-management skills.

• Excellent communication and customer service skills.

• Ability to work in a fast-paced environment and adapt to changing priorities.

JOB SUMMARY

The Home Health Scheduler is responsible for coordinating and scheduling home visits for nurses, therapists, aides, and other clinical staff. This role ensures patient care is delivered efficiently by optimizing staff schedules, responding to scheduling changes, and maintaining accurate documentation while providing excellent customer service to patients and healthcare professionals.

KEY RESPONSIBILITIES

• Schedule and coordinate patient visits for nurses, therapists, and home health aides.

• Assign clinicians based on availability, geographic location, patient needs, and skill level.

• Monitor daily schedules and make adjustments for cancellations, emergencies, or staff call-outs.

• Communicate scheduling changes promptly to patients, caregivers, and clinical staff.

• Maintain accurate scheduling records in the electronic medical record (EMR) system.

• Verify patient information and ensure physician orders and authorizations are in place before scheduling visits.

• Coordinate with clinical managers to ensure staffing needs are met.

• Track missed visits and reschedule as appropriate.

• Answer incoming calls and assist patients, caregivers, and staff with scheduling inquiries.

• Ensure compliance with agency policies, HIPAA regulations, and Medicare/insurance requirements.

JOB CONDITIONS

• Position is stressful in terms of meeting deadlines.

• It is primarily a desk job, which essentially involves sitting, standing, stooping, and walking, as well as an inordinate amount of telephone communication.

• It requires minimal lifting of office records and printouts.

• The ability to read 12 point, and larger, type, is required.

• One must be able to adequately hear, on the telephone, with no more than an amplifier and be able to communicate, both, verbally, and in writing, in English.
