Pharmacy Medication Reconciliation Technician Job Summary The Pharmacy Medication Reconciliation Technician will improve medication safety by assisting pharmacists in the collection and documentation of up-to-date medication lists for patients being admitted to the organization’s hospitals in order to optimize the medication reconciliation process and reduce the number of medication errors secondary to inaccurate or incomplete information. Responsibilities Obtain and verify medication history in the electronic medical record accurately and timely in compliance with standard procedures and regulatory standards. Accurately document medication lists into the electronic health record, communicate to the verifying pharmacist, and relay any outstanding transition of care issues to the pharmacist and/or provider in a timely manner. Perform other duties as assigned. Education High School Diploma or GED Required Completion of a formalized Technician Training Program Preferred Experience 10+ months pharmacy technician experience or completion of a formalized technician training program Required Retail pharmacy experience and/or excellent customer service skills Preferred License and Certifications Certified Pharmacy Technician (CPhT) – Current license by AZ State Board of Pharmacy Required Certified Pharmacy Technician (CPhT) – Current Pharmacy Technician Certification issued by Pharmacy Technician Certification Board (PTCB) Required Pharmacy Technician (LPT) – License or Pharmacy Technician Diploma Preferred #J-18808-Ljbffr HonorHealth
