Family Support Coordinator

Communitas, Inc., a mission-driven human services organization in Somerville, MA, seeks a Family Support Coordinator to partner with families of individuals with intellectual and developmental disabilities. This role provides person-centered support, helping families access resources, navigate services, and develop individualized support plans that promote inclusion, independence, and dignity. The Coordinator conducts home and community visits, connects families to community programs and benefits, coordinates with internal and external providers, and maintains accurate documentation. You will collaborate with a compassionate, client-focused team and receive training, supervision, and growth opportunities in human services and advocacy.
