Family Practice/Primary Care Physician Assistant

New locum in Willow Creek, CA for Family Medicine NP/PA
Scheduled clinic hours with no call requirement
Licensed in CA only at this time
Candidate CVs need to have month & date on their work experience w/all gaps in employment accounted for.
Chart closure within 72 hours
Schedule = 18 PPD, minimum is 18 PPD
Monday-Friday 8-5
Mix of peds/adults
A decision is typically made within 7-14 days
APC's with 3 years of experience in similar setting
PAs must have CA Controlled Substance Education Course Certificate
NPs must have Furnishing License with Schedules II
Active DEA needed before start
Credentialing takes 30-90 days
The following immunizations are required for credentialing (before start date): Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
HCO & Vendor agree to a 30-day notice for ending all assignments.
