We are seeking a skilled and compassionate Occupational Therapy Assistant to join the Parkview Health at Home team. The OTA will work collaboratively with the OT and other interdisciplinary team members to plan, implement, and evaluate occupational therapy services for patients in their homes.Responsibilities:Assess patients' physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises, adaptive techniques, and use of assistive devices to improve patients' functional abilities.Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.Monitor patients' progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.Collaborate and communicate patient's change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.Job Requirements:Associates of applied science (AAS) degree in occupational therapy assistant studies or a related field.Active registered OTA license in the state which you practiceMust obtain and maintain current CPR/BLS CertificationOne-year Occupational Therapy Assistant experience in a clinical setting.Current Drivers LicenseComply with federal, state and local laws and regulationsReady to Make a Difference?At CommonSpirit Health at Home, we are proud to be an Equal Opportunity Employer, promoting diversity, equity, and inclusion in every aspect of our organization.
