divh2We Are Seeking Your Skills And Compassion As An MDS Nurse To Join Our Team!/h2pAt Bear Mountain Healthcare we pride ourselves on the level of professionalism our team exhibits every day in the care of our resident family./ph3Primary Responsibilities:/h3ulliCompletes assessments, Minimum Data Set (MDS) and care plans for all residents assigned/liliMonitors completion of MDSs by other disciplines within timeframes prescribed by regulatory guidelines/liliAdvises supervisor of incomplete and/or untimely assessments by disciplines other than nursing/liliEnsures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents/liliInitiates care plans and supporting activities that will result in best possible outcome for assigned residents/liliGenerates and distributes monthly care plan calendar for the following month/liliMaintains and updates all care plans and assessments for assigned residents on a quarterly basis (at minimum) and adds/deletes issues as necessary/liliSupports and promotes facility and company standards for superior customer service by exhibiting positive, courteous, and helpful behavior when dealing with all internal and external customers/liliComplies with established standards described in facility policies and procedures, code of conduct, corporate compliance plan, employee handbook and other company documents and publications/li/ulh3Minimum Job Requirements/h3ulliMust hold a current LPN/ RN license in MA/liliMust be fully vaccinated per MA regulations/li/ulh3Benefits Include:/h3ulliCompetitive salary and wages/liliMedical/Dental/Vision/liliSick/Vacation/PTO/lili401K/liliLife Insurance/li/ul/div
