Registered Nurse (Case Manager)The case manager performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client's care.Assess new patientsUpdate and revise patient health care plans as neededEducate patients and families about health care plans and optionsMake clinical decisions for routine patient careAccurately document and submit medical recordsDemonstrate knowledge in medical care; such as diagnostic procedures, medication, symptoms, and other treatment-related therapiesProvide emotional support to patients and familiesEvaluate and determine needs of patientsReview results from medical testsImplement physician orders and other nursing proceduresCreate an environment for open communication2 years of Acute Care experience (can consider 1.5 years)Experience with Utilization Review and Discharge PlanningExperience with InterQualCalifornia Registered Nurse LicenseBLS-Basic Life Support
