Job Opportunity

Overview

We are seeking a qualified professional to join our team at an acute care facility located in City, State . This position offers a day shift, working a 5x8-hour schedule from 08:00 AM to 4:30 PM. The ideal candidate will be experienced in acute care case management and discharge planning.

Key Responsibilities

• Manage a caseload of up to 25 patients.
• Conduct assessments and utilize InterQual criteria in the admission and discharge planning process.
• Organize and lead family meetings to discuss treatment plans.
• Provide care coordination and determine medical necessity based on evidence-based guidelines.
• Oversee discharge planning and ensure appropriate post-acute care arrangements including skilled nursing facilities and home health services.
• Participate in interdisciplinary collaboration with various teams, including Social Services, Physical Therapy, and Rapid Response Teams.
• Assist in managing appeals and denials as related to patient care and insurance.
• Ensure compliance with CMS regulations and HIPAA guidelines.

Qualifications

• Active CA state license; pending licenses accepted, but must be licensed upon start date.
• Minimum of 2 years of experience in acute care case management or discharge planning.
• Experience with charting systems, specifically Epic.
• Understanding of Medicare, Medi-Cal, and other insurance processes.
• Previous charge experience is preferred.
• Ability to work every other weekend and accommodate holiday shifts as required.

Job Requirements

• Strong knowledge in areas such as behavioral health, disease management, and advance directives.
• Competency in managing medical necessity evaluations and prior authorizations.
• Experience with diverse patient populations including neonates, adolescents, adults, and geriatrics.
• Ability to work independently and effectively under pressure.

Additional Information

This position may require floating between different facilities within the state. All travelers are expected to adhere to this policy. Communication devices, if required for the assignment, will be provided at no cost.
