RN, Admissions (11am-10pm) LifePath Hospice

Position Overview The Admission Registered Nurse (RN) assesses patients in their place of residency or in hospitals and initiates care into the services of Chapters Health System. The RN ensures a seamless transition to Care Navigation and serves as an ambassador of the organization. Qualifications Current RN license in the state where the employee will be working. Minimum of one year nursing experience; hospice or hospital experience preferred. Prior experience with an EMR/EHR (Electronic Medical/Health Record) system. Strong clinical assessment skills, preferably with a background in emergency department or ICU. Ability to effectively communicate in English (verbal and written); bilingual in Spanish preferred. Ability to present services in a professional and articulate manner. Valid driver’s license and automobile insurance required for mobile driver role. Reliable transportation to meet schedule. Ability to work in a team environment and intervene appropriately. Ability to manage multiple priorities simultaneously and handle the emotional stress of the workload. Strong goal/outcome orientation with exceptional time‑management skills. Excellent customer service focus. Active Basic Life Support (BLS) certification for healthcare professionals from the American Heart Association or Red Cross. Responsibilities Complete clinical admissions for all company services, including explaining services to potential patients/families and addressing their concerns. Identify admission objections and provide appropriate responses. Educate patients/families on admission requirements and immediate needs. Complete informed consents and initial assessment in accordance with regulations and company policy. Provide information to physicians and initiate a plan of care to address patients’ immediate needs. Initiate skilled nursing interventions to prevent complications, alleviate symptoms, and maximize physical and emotional comfort. Obtain physician orders and complete documentation per company policy. Represent the company at assigned facilities and facilitate referrals to all service lines. Communicate frequently with Care Navigation staff to ensure timely follow‑up and inform the team when assignments are complete. Use SBAR for all necessary clinical communication. Discuss potential needs with after‑hours staff. Obtain consents for services and arrange safe discharge efficiently for referred and eligible patients while meeting productivity expectations. Obtain medical record information for the referral. Develop strong relationships with case managers, physicians, and other facility staff. Collaborate with Professional Relations Representatives (PRRs) to provide education on all available services. Provide care for any GIP patients in a hospital, including daily visits, professional management, documentation, and efficient discharge. During emergencies (e.g., hurricanes), the Triage RN may be required to report to a designated location at the company’s directive, potentially including working ahead of scheduled time and overnight stays based on the emergency duration. Perform other duties as assigned. Physical Demands Ability to see, hear, talk, walk, and use hands to finger, handle, or feel. Frequently required to stand, sit, reach with hands/arms, lift, and bend. Occasionally required to pull, push, stoop/crouch, kneel, and climb stairs.
