Clinical Pharmacist Lead

We are seeking a detail-oriented and collaborative Clinical Pharmacist Lead to support safe and effective medication use within the organization. Key responsibilities include: Verifying medication orders and collaborating with nursing staff Managing inventory, medication preparation, and Pyxis refills Supporting medication delivery, patient counseling, and prior authorizations Participating in REMS programs and departmental committees Contributing to workflow optimization, policy development, and patient safety initiatives Supervising and directing pharmacy support personnel Evaluating medication appropriateness based on patient condition Providing education and serving as a liaison with healthcare practitioners Leading in policy review, implementation, and process improvements Qualifications and skills: Bachelor’s Degree in Pharmacy; Licenses including RPh and Medication Therapy Services in MO 2-5 years of experience; supervisory experience preferred Strong clinical knowledge, teamwork, and leadership abilities Ability to assess, monitor, and modify medication therapy based on patient needs Participation in educational initiatives and policy development This role offers an opportunity to contribute to a high-performing team focused on quality patient care in a dynamic clinical environment. #J-18808-Ljbffr LiveWell Homecare Agency, LLC
