We’re supported by top, mission-driven VCs to empower families throughout the country. The Role We are seeking a dedicated and experienced professional to join us as PASA Case Manager . This role will report into the PASA Program Lead – Colorado. This is a Full‑Time Hybrid position based in Denver, CO. Key Requirements 2-3 years of direct experience as a case manager or similar role specifically within Colorado's PASA program (CES/SLS Waivers). Familiarity with PASA services such as Community Connector, Homemaker Enhanced, Respite, or Supported Community Connector. Experience in care coordination, patient onboarding, caseload management, and Medicaid home health compliance. Strong organizational skills with proficiency in Google Workspace, Salesforce, or equivalent tools for documentation and reporting. Excellent communication and empathy, with the ability to support diverse families in a person-centered approach. Ability to work independently in a fast-paced, hybrid environment while thriving in ambiguity. Located in the Denver metro area; willing to travel across Colorado as needed. Bachelor's degree in social work, nursing, healthcare administration, or related field (preferred).
