LPN

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team in Washington Township, OH. Kauhale Centerville is a memory care community. The LPN will provide high-quality nursing care to patients, assist with treatment plans, and support the healthcare team in delivering excellent patient outcomes.

Key Responsibilities

• Provide direct resident care under the supervision of Director of Health and Wellness.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and record patients’ vital signs and health status.
• Assist with patient assessments and report changes in condition to the healthcare team.
• Maintain accurate and detailed patient records and documentation.
• Educate patients and their families on health maintenance and disease prevention.
• Ensure compliance with healthcare regulations and facility policies.
• Collaborate with interdisciplinary team members to coordinate patient care.

Qualifications

• Current and valid LPN license in the state of Ohio.
• Completion of an accredited practical nursing program.
• Previous experience in a clinical or healthcare setting preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively both independently and as part of a team.
• Basic computer skills and familiarity with electronic health records (EHR) systems.
• CPR and Basic Life Support (BLS) certification preferred.

This position is based in Washington Township, OH, providing care in a fast-paced healthcare environment. The role may require standing for extended periods, lifting or moving patients, and working various shifts including weekends and holidays.
