Embark on a career that fulfills more than just professional goals—it transforms lives every day.About This Role:Care Coordination & Case Management Coordinate care for patients and families by connecting them with providers, specialists, behavioral health services, and community resources.Develop, monitor, and update individualized care plans to support patient-centered goals and improve health outcomes.Patient Education & Engagement Provide individual and group education on behavioral health, chronic conditions, healthy behaviors, and available Evara Health services.Engage patients and families in their care, helping them navigate services and overcome barriers to treatment.Population Health & Quality Improvement Identify high-risk patients, monitor progress, and collaborate with care teams to ensure patients receive the right level of care at the right time.Assess service utilization, identify gaps in care, and support initiatives that improve quality, continuity, and patient outcomes.Collaboration & Team Support Partner closely with providers, therapists, nurses, and care team members to coordinate comprehensive patient care.Participate in huddles, case reviews, committees, and workflow improvement initiatives while serving as a resource for staff and community partners.Documentation & Compliance Maintain accurate documentation, manage assigned tasks, and ensure timely follow-up in accordance with Evara Health policies.Deliver exceptional customer service while adhering to all HIPAA, State, Federal, and organizational requirements.Why You'll Love Working Here:Impact: Every day, you'll make a significant impact on our patients' lives, leading efforts that go beyond healthcare to ensure community wellbeing.Growth: We support your professional development through continuous learning and opportunities to grow within Evara Health.Recognition: As part of our team, your hard work will be recognized and rewarded, contributing to your professional fulfillment and job satisfaction.Education & Experience:Graduate of an accredited RN Program Valid and active RN license in the state of FloridaRequired: Minimum two (2) years as an RN in a healthcare setting with some supervisory experiencePreferred: Minimum two (2) years working with pediatric populationPreferred: Certified Case ManagerCulture and Benefits:What sets Evara Health apart is our amazing culture and team spirit.
